Special Requirements
Group & Hold and Crossmatch specimens can only be held for 3 (three) days. BUT if the patient is NOT pregnant or has not been transfused in the last 3 months the specimen can be kept for 1 (one) month. REQUEST FORM: Must contain the following information - The Date and Hospital where the transfusion/operation will take place. - If the operation/transfusion is more than 2 (two) days later check if the patient is pregnant or has been transufsed in the last 3 (three) months. - Request Form must be signed by both the Doctor and The Collector. SPECIMEN: Must be labeled with the following information. - Patient Surname, First Name and Date of Birth/UR Number. - Date and Time of collection. - Signature of Collector.
